Ford S-Max 1.8 TDCi Fault: "high fuel consumption" (and a strong smell of diesel)

Description

Car was returned by a Ford dealer in this potentially lethal condition, after investigation by a Ford Technical engineer for a strong vibration issue. After driving a mile the smell diesel in the cabin was overwhelming. The source of the stink was traced to the engine compartment, where the 3rd injector from the left was observed leaking diesel all over the engine.
