Ford S-Max Fault: 1.8 TDCi shuddering when accelerating in 4th gear

Description

Ford S-Max 1.8 TDCi engine fault. Have a listen. The whole car shudders like a wobbly spin drier when accelerating in 4th gear from 35 mph to 45 mph at full throttle (1700 rpm to 2000rpm). This fault manifests after 10 miles (or more) of motorway driving at a constant 70mph. ( Video contains very low frequency audio - best with a subwoofer or good headphones)
